Privacy
Our privacy policy is simple: Skin Care By Design will not reveal any information about our clients to anyone except to comply with a valid legal or law enforcement process. This includes all personally identifiable information provided or collected on this website and at our practice.
Personal information such as telephone number, postal address, and e-mail address is used internally for maintaining records, confirming appointments, and post-treatment followup. We occasionally send postcards or e-mails with information about our services or promotions but we do not conduct any form of telephone marketing and we do not sell, exchange, or distribute name, address, telephone, or email lists. You may contact us at (520) 797-7546 to be added to or removed from our mailing list.
If you want treatment information forwarded to another person or practice, you must sign a Release Authorization allowing us to do so. For example, if you undergo a series of annual Skin Cancer Screenings and a skin growth or lesion becomes suspicious, you could request that we forward all photographic and clinical documentation to your dermatologist to aid in any diagnosis.
The Health Insurance Portability and Accountability Act of 1996 (HIPAA) requires us to provide you in writing with our policy concerning the protection of your medical information. This notice describes how information about you may be used and disclosed and how you can get access to this information. Top of pageArrivals and Cancellations
On your first visit, we ask that you arrive 20 minutes early to fill out a three page Skin Care History form. The purpose of this form is to obtain information that helps determine which treatments and products are best suited to your unique skin. Doing this in advance of your appointment allows us to devote the entire appointment to answering your questions and providing the services you request.
Please call us as soon as possible to cancel or reschedule an appointment that you cannot keep. Clients seeking appointments when our schedule is full frequently ask us to call them if an appointment is cancelled so we ask that you afford us the simple courtesy of a phone call that you would expect from someone who has reserved your own valuable time. Our telephone system allows you to leave messages outside our normal hours of operation.
If you are late for an appointment and do not contact us, we may begin services for waiting clients. We will do everything possible to accommodate late arrivals but your appointment may need to be delayed or rescheduled.

We accept the following forms of payment:
- US currency
- Credit cards from Visa, MasterCard, American Express, and Discover
- Debit cards with a Visa or MasterCard logo
- Skin Care By Design Gift Cards
- American Express Travelers Cheques
- Personal checks. Checks from outside the Tucson, AZ metropolitan area must be guaranteed with an accepted credit card. Returned checks are subject to a $25 fee.
Skin Care By Design does not accept or process insurance claims. Most of our services are not covered by insurance but we will provide treatment documentation to any clients who wish to submit their own insurance claims for reimbursement.

Skin Care By Design accepts the return or exchange of unopened, undamaged products within ninety days of the original purchase. A container is considered to be open if the safety seal is broken and may not be returned or exchanged.
We gladly provide samples of most of our products prior to your first purchase. This allows you to ensure that the product characteristics are acceptable and well tolerated before purchasing the product.
Products purchased "on sale" with sale displays or ads that clearly show "nonreturnable" or "all sales final" may not be returned or exchanged.

No Smoking
Smoking is not permitted anywhere on the premises of Skin Care By Design.

Our nurses, receptionists, and office staff are professionals who provide exceptional service to all of our clients and do not accept tips. However, Skin Care By Design supports important causes such as breast cancer research and disaster relief. If you are delighted with our service and notice a contribution receptacle in the lobby, please join us in supporting those causes in lieu of a gratuity.

We perform many services that are appropriate for children. However, everyone under the age of 18 receiving services from Skin Care By Design must be accompanied by a parent or legal guardian on each visit and the parent or legal guardian must sign the appropriate Informed Consent forms for each visit.
We request that clients receiving services do not bring children under the age of 13 to their appointment unless the children are accompanied by another adult.
