• Within 14 days after your first Dysport treatment, you must sign up for the Dysport Challenge, download your Dysport Coupon Rebate Form, and mail the rebate form and itemized receipt. You can get a $75 Rebate Check whether or not you have a second treatment.
• Within 15-30 days after your first Dysport treatment, return to the Dysport challenge website to decide if you Love It or Leave It. If you love it, Dysport will give you another $75 rebate on your second Dysport treatment to the same area. If you leave it, Dysport will give you a $75 rebate on your next Botox treatment to the same area. You must wait a minimum of 3 months between your first and second treatments.
